At Tripple S, our qualified and experienced Specialist Support Coordinators work closely with participants to navigate complex challenges that impact their ability to access and maintain the right supports. In some cases, a tailored service plan may be required to manage significant complexities in a participant’s life, ensuring they receive the support they need.
A Support Coordinator is someone who helps people with disabilities or other complex needs connect to the services and supports that will help them live more independently. We work closely with you as well to understand your unique goals and challenges, then assist you in putting together a plan that brings together the right resources. Essentially, a Support Coordinator is a friendly and reliable point of contact, helping you get the services you deserve while promoting your independence and well-being.
At Tripple S, our Specialist Support Coordinators collaborate with participants, their families, and carers to identify a wide network of funded supports that enhance the effective use of their plan. We develop tailored strategies to address challenges, achieve goals, and navigate crisis situations. Additionally, we assist participants with:
- navigating NDIS systems and processes
- establishing their supports, including negotiating with providers
- empowering individuals to choose and manage their supports in a way that best suits their needs, ensuring they get the most out of their plan.
Supporting Participants in Plan Implementation
When implementing supports within a participant’s plan, a Support Coordinator plays a key role in ensuring the plan is effective and aligned with the participant’s evolving needs. This includes:
- Assessing how well supports have met, and continue to meet, the participant’s needs.
- Monitoring progress towards achieving the participant’s goals and evaluating how current supports contribute.
- Adjusting the mix of supports or their delivery to reflect the participant’s preferences and requirements.
Preparing for Plan Reassessment
To support an upcoming plan reassessment, we provide comprehensive evidence, including:
- Successes and achieved outcomes.
- Areas the participant wishes to explore, modify, or enhance in their next plan, along with required supporting evidence.
- Progress in skill development and independence.
- Advancements in community and economic participation through broader support systems.
- Implementation and utilization of NDIS-funded supports.
- Relevant reports from therapists, providers, or other funded services submitted to the NDIA.
- Identification of barriers, risks, or challenges in plan implementation, along with strategies used to address them.
- Confirmation that a crisis or complex service plan has been developed and implemented if necessary.
- Clear evidence of future support needs and recommendations.
